It doesn't really count as a game over, per se, but when you almost reach the top of the mountain in Mario and Luigi Dream Team and drink from the fountain. That crap sent ice cubes down my back.
I can't name any ones that spooked me off of the top of my head, but if I had to choose one, it would be Donkey Kong Country 2: Diddy's Kong Quest. The imagery of Diddy & Dixie being left to rot in some sort of jail coupled with the depressing music really gives off an unnerving vibe to me.

Tenchu: Wrath of heaven. Each character has a unique game over screen that gives a small poem that seems to be their last thoughts before they die. Still creeps me out.

In Spec Ops: The Line, there is no such thing as a good ending. I'll leave it at that. It still hangs over my head. At least one of the endings can be seen as a 'game over.'
Chrono Trigger's 'But the future refused to change' really brings the nihilist out of you. Just a super depressing message of 'you failed and in the grand scheme of things you amounted to dust'.
I know Bowser's laugh in Super Mario 64 always scared me back then....I wish it was back in Super Mario Odyssey!![]()

Dying in Spyro 2 was honestly kinda creepy...No music or sound, just him over a cliff
RE1, especially when you die to the giant snake, or die to a leaping hunter, or became a Jillsandwich and then the cheesy line stuck in your head...
The one that disturbed me the most in my life was Ghosts 'N Goblins on the NES. It's just something about the sudden cut to text over a black screen with either blaring diabolical music or complete silence that still makes me uneasy today.

i'd embed a video of it if i knew how... but basically you get a picture of a skeleton in the desert, with a demonic voice saying "Game Over... Muhahahaha!" first time that happened i was like "oh sh**, if Alucard fails, the world is doomed!"
after a few times though that novelty runs out as you wait to get to load your save...
